Below is a list of documents you will need to bring with you when you attend an identity interview:
Proof of Identification
- National Identity card (UK or foreign)
- Valid Passport (UK or foreign)
Proof of address
- Full birth or adoption certificate
- Residence permit or residence card including biometric immigration residency documents
- Full marriage or civil partnership certificate
- Driving licence (UK or foreign)
Lost National Insurance Number Card
If you have lost your National Insurance number you may be able to find the number either on your payslip, P60 or on a tax return. If you still can’t find the lost National Insurance number you can receive a replacement. However, this will be sent to you in the post and will not be given over the telephone.

Please note that the HMRC no longer issue a National Insurance number card, therefore you will receive your National Insurance number on letter headed paper directly from the HMRC. When completing the form online for your lost National Insurance number you will be required to enter your information including full name and any previous names, full current address as well as your previous address if applicable, your date of birth and any marriage details including date of marriage.
Your National Insurance number will consist of 2 letters, 6 numbers and 1 letter at the end and will look something like this (an example): AC654321C. Your National Insurance number will never change and you will keep this number for the duration of your life. When you have received a copy of your lost National Insurance number it is important that you keep this number safe and do not disclose it to anyone who does not need it. If you have started employment and have lost your National Insurance number then your employee should allow you time to apply for a replacement and this shouldn’t stop you from starting work.

When working in the UK it is law that you pay a certain amount of National Insurance and Tax to the government in relation to your circumstances and the amount you earn.
